Welcome to the Patient Enrollment Review Guide on the Develop Health platform. This guide walks you through the “Submit Patient Enrollment” page, detailing its key features and capabilities. Navigating these features efficiently can ensure a smooth enrollment experience for new patients.

Key Features:

  1. Patient Information:

    • Presents vital data about the patient:

      • First Name
      • Last Name
      • Address
      • Birthday
      • Phone
      • Unique Patient ID
    • Autofilling Capability: This added functionality can detect and populate fields with available patient data, ensuring faster processing and fewer manual entries.

  2. Program Details:

    • Gives an overview of the patient support program they are enrolling in:
      • Program Name
      • Duration
      • Key Benefits
  3. Required Documents:

    • This section lists all essential paperwork and consents needed for enrollment. The platform can directly fax these to the patient support program, simplifying documentation procedures.
    • Autofilling Capability: This added functionality can detect and populate fields with available patient data, ensuring faster processing and fewer manual entries.
  4. Additional Enrollment Queries:

    • Asks specific questions to ensure the patient meets all criteria for the support program. The system provides guidance and context for each question, ensuring clarity and accuracy.

  5. Confirm and Proceed:

    • Situated at the end, this button confirms the enrollment details and sends them off for processing.

Using the Patient Enrollment Review Page:

  1. Review Enrollment Alerts: Begin by evaluating the enrollment status alert to grasp any potential hurdles in the process.

  2. Double-Check Patient Information: Verify the accuracy of all patient details. Leverage the autofilling functionality to streamline data entry.

  3. Survey Program Details: Familiarize yourself with the specifics of the patient support program the patient wishes to join.

  4. Manage Documents: Ensure all mandatory documents are uploaded. Use the faxing feature to forward these directly to the patient support program if needed.

  5. Tackle Enrollment Queries: Rely on the platform’s guidance to answer each question effectively and comprehensively.

  6. Finalize: Once all sections are duly completed, hit the “Confirm and Proceed” button to submit the enrollment details.
